How to make the page fold effect, rather than the page curl (or vice versa)?

To change the type of the effect, just click the appropriate radio-button (Curl or Fold) on the Surface panel of the plug-in's Main window. See Fig.1 and Fig.2. Then use the controls of the plug-in to create the desired effect. Read the User Guide to learn how to use these controls.
Pay attention to the Details.
